Physical Examination

Physical examination are good for 12 months from the last date of Physical Examination. Separate signed form is required for each school year MAY 1 to APRIL 30.   YOU MUST HAVE A PHYSICAL ON FILE TO TRYOUT OR PARTICIPATE IN PRACTICE OR GAMES.
